Madhavan's Victory: Defeating Brain Glioma with Bioocus B7H3 CAR-T Therapy

Patient Background

Madhavan was diagnosed with brain glioma, a malignant tumor known for its invasiveness and poor prognosis. At 58, after conventional treatments including surgery and radiation failed, he turned to Bioocus for advanced options. Referred to Tianjin Tumor Hospital Konggang Hospital, he enrolled in our B7H3 CAR-T program under treatment number P001. B7H3 is highly expressed on glioma cells, making it an ideal target for engineered T cells administered via cerebrospinal fluid (CSF) to directly attack the tumor.

The Treatment Journey

Madhavan's therapy involved infusing autologous CAR-T cells into the CSF, with monitoring through serial CSF analyses from Day 2 (D2) to Day 28 (D28).

• D2 (August 8, 2025):Strong initial presence with B7H3 CAR-T at 54.44% of CD3+ T cells.
• D4 (August 11, 2025):52%, reflecting early dynamics.
• D7 (August 14, 2025):Surged to 59.51%.
• D10 (August 17, 2025):12%.
• D14 (August 21, 2025):19%.
• D21 (August 28, 2025):Peaked at 89.69%, showing exceptional expansion.
• D28 (September 6, 2025):Stabilized at 13.58% of CD3+ T cells, with 11.90% in CD4+ and 14.28% in CD8+ subsets. Overall, CD3+ T cells dominated at 95.34%, with high CD8+ (66.74%) indicating potent cytotoxicity.

Expansion graphs illustrated fluctuating but overall robust CAR-T persistence, essential for sustained tumor control in the CNS.

Outstanding Outcomes

The high peak expansion on D21 correlated with tumor shrinkage on MRI, leading to a partial response by D28 and continued improvement. This intrathecal delivery minimizes systemic side effects while maximizing efficacy against gliomas, with our data showing improved progression-free survival. Madhavan reported manageable symptoms and no severe neurotoxicity.
