PCNSL is most often seen in patients over the age of 60, with a rising incidence in older adults. The disease typically presents with symptoms such as confusion, headache, and focal neurological deficits, though seizures are rare. Magnetic Resonance Imaging (MRI) is still the gold standard for diagnosis, but challenges remain in distinguishing PCNSL from other brain tumors and conditions due to overlapping features.

The review also emphasizes the role of advanced diagnostic techniques, such as deep learning models in MRI analysis, to improve diagnostic accuracy. Furthermore, the importance of genetic mutations like MYD88 L265P is highlighted as a potential biomarker for diagnosing and monitoring disease progression, offering a new pathway for more precise treatment.

 

For treatment, high-dose Methotrexate (HD-MTX) remains a cornerstone, often combined with whole-brain radiation therapy (WBRT). However, research continues to explore the effectiveness of reducing radiation doses to minimize neurotoxicity while maintaining progression-free survival (PFS). New approaches in chemotherapy and potential adjunctive treatments like Glucarpidase show promise in reducing toxicity and improving outcomes.

 

PCNSL is a complex and aggressive disease, but ongoing research is opening up new avenues for treatment and improving the overall prognosis for patients. As these advancements continue, the hope is that survival rates and quality of life for PCNSL patients will improve significantly.
