Acute Lymphoblastic Leukemia(B-ALL)-01

Nadayagnos nga adunay acute B-cell lymphoblastic leukemia niadtong Nobyembre 28, 2017.

Pagtambal sa VDLP regimen sa sinugdanan, pagkab-ot sa partial bone marrow remission (mga detalye wala gitaho).

Pebrero 2018: Gibalhin sa VLCAM regimen. Ang bone marrow flow cytometry nagpakita sa 60.13% nga malignant immature B cells.

Marso 2018: Na-enroll sa BiTE clinical trial. Morphological remission sa bone marrow, walay malignant immature nga mga selula nga nakita sa flow cytometry.

Mayo 8, 2018: Nakadawat ug TBI/CY+VP16 conditioning regimen nga gisundan sa allogeneic stem cell transplant gikan sa fully matched nga igsoon (AB+ donor ngadto sa A+ recipient). Neutrophil recovery sa adlaw +11, megakaryocyte recovery sa adlaw +12.

Morpolohiya sa utok sa bukog: Hypercellular (grade II), 96% immature lymphoblasts.

Immunophenotyping: Ang mga selula nagpahayag sa CD19, cCD79a, CD38dim, CD10bri, CD34, CD81dim, CD24, HLA-DR, TDT, CD22, CD72; partial nga ekspresyon sa CD123. Giila nga malignant immature B lymphoblasts.

Mutation sa tumor sa dugo: Negatibo.

Leukemia fusion gene: NUP214-ABL1 fusion gene positibo.

Pagtuki sa Chromosome: 46, XX, t(1;9)(p34;p24), add(11)(q23)[4]/46, XX, t(1;9)(p34;p24), add(11) (q23)x2 [2]/46, XX[3]

Chimerism: Ang mga selula nga nakuha sa donor adunay 7.71%.

Pagtambal:

- VDS, DEX, LASP chemotherapy regimen nga gipangalagad.

- Nobyembre 20: Ang peripheral blood immature cells 0%.

- Gidaghanon nga pagsusi sa NUP214-ABL1 fusion gene: 24.542%.

- Flow cytometry: 46.31% sa mga selula nagpahayag sa CD38dim, CD22, BCL-2, CD19, CD10bri, CD34, CD81dim, CD24, cCD79a, nga nagpaila sa malignant immature B lymphoblasts.

- Disyembre 4: Pagpuga sa autologous CD19/22 dual CAR-T cells (3 x 10^5/kg).

- Mga side effect nga may kalabotan sa CAR-T: Grade 1 CRS, hilanat sa Ika-6 nga Adlaw nga adunay Tmax nga 40°C, kontrolado ang hilanat sa ika-10 nga Adlaw. Walay neurotoxicity nga naobserbahan.

- Disyembre 22 (Day 18 assessment): Morphological complete remission sa bone marrow, walay malignant immature cells nga namatikdan sa flow cytometry. Ang quantitative assessment sa NUP214-ABL1 fusion gene: 0%.
